NDIS Review Reports
NDIS reports can be completed by clinicians at Glow Speech Pathology and Services. We ensure to identify support needs and do this confidently by knowing the participants well through direct discussions with the participant, family and supports as well as building a strong connection throughout our time together. In our NDIS review reports these will include:
name, date of birth, address, NDIS number etc.
Introduces participant from information gained during clinical case history information, observations and intervention
Is information that has been asked to pass on directly to the NDIS from the participant
Details how funds have been used clinically by participant and clinician
Lists goal, therapy input to address these goals, COPM score and if goal is ongoing
Things that have helped and hindered intervention
Outlining future NDIS funding for speech services, breakdown of hours, justification and other services required
Potential additional goals, other considerations and attached reports
